Why do they dispute over carry-on items?

It seems to be difficult to define items that should be excluded from counting as carry-on items. But, when we consider that items which are necessary for passengers to walk must be excluded from counting, it is not difficult.
For example, if we think about if this shillelagh is necessary for passenger whose foot is disabled, it would be easy to get answer.
